Care coordination services for people living with HIV or other health issues to ensure access to medical care, treatment, insurance coverage, dental care, and linkage to other needed resources such as mental health, housing and substance use disorder treatment.
Non Medical Case Management
​
Case Managerment services are for people living with HIV or other health issues that are connected to medical care but need short-term assistance such as transportation to medical appointments, medication, or insurance coverage.
